Diabetes Educator/Lifestyle Coach

Location:  All 50 States

Description:  Provide individualized diabetes self-management education and training.

Qualifications:

  • Must be 18 years of age or older
  • Certified Diabetes Educator (CDE) with experience in healthy psychology and motivational interviewing
  • Ability to communicate effectively with participants of various cultures and backgrounds
  • Ability to adhere to accepted medical guidelines/practices when providing health education
  • Friendly, professional manner

Key Responsibilities:

Support lifestyle modifications via telephonic coaching for successful management of daily diabetes routines such as:

-          Self-care strategies

-          Monitoring blood glucose

-          Taking medications as prescribed

-          Healthy food choices

-          Physical activity

-          Regular medical care
